Guidelines for Add-on Dependent (Spouse/Children/Parents) Application
To embark on your MM2H journey with family, additional requirement are as following:
Required Documents:
- Applicant’s Presence:
Applicant or representative MM2H companies must be present. - Letter of Intent:
Intention letter for individual / agency representative letter for MM2H company (if relevant). - MM2H Agency Director’s Details:
Copy of MM2H Agency Director NRIC (if relevant). - Passport Details:
Original and copy of the main applicant’s passport (particular and MM2H pass page).
Original and copy of the dependent’s passport (particular page). - MM2H Pass Copies:
Copy of the main applicant’s MM2H pass.
Copy of the dependent’s passport page bearing the most recent entry stamp. - Imm. 12 Form for Dependent:
Form Imm. 12 with photo (blue background) for the dependent. - Family-related Documents (if applicable):
Attested copy of marriage certificate/birth certificate (if add-on children).
Attested copy of parent’s marriage certificate (if add-on both parents) / main applicant’s birth certificate (add-on parents) OR marriage certificate and spouse’s birth certificate (add-on parent-in-law). - Official Relationship Documents:
Relationship documents issued by foreign authorities, such as marriage certificates and birth certificates (if not in English, must be officially translated into English) and authenticated by either the Malaysian Embassy or Consulate abroad or the Foreign Embassy or Consulate in Malaysia. - Conditional Approval Documents:
Original and copy of conditional approval letter. - Health Insurance:
Original and copy of health insurance policy and payment receipt (Dependent below 60 years old). - Medical Report:
Medical Report Form MFII (Dependent). - Personal Bond:
Stamped personal bond.
